Liability Waiver for Latin Roots
I acknowledge the inherent risks of injury in dance activities and assume those risks. I release Latin Roots, its employees, and dance instructors from all liabilities for any injuries or illnesses incurred during participation in any activities. I will indemnify and hold harmless Latin Roots for any damages resulting from my conduct.
Personal property: I am solely responsible for my personal property and release Latin Roots from liability for any loss or damage.
Photo & Video Notice:
At Latin Roots, we occasionally capture photos and videos during classes and events to celebrate our community and promote the academy. These may be used on our social media channels, website, and other promotional materials.
By attending Latin Roots activities, I understand that I may appear incidentally or as part of a group in this content. Latin Roots relies on legitimate interests as the legal basis for creating and using this material.
If I prefer not to appear in future recordings, I can inform the team. This notice applies to new recordings going forward and does not require Latin Roots to remove images or videos already published.
Purchase Policy: I understand the commitments involved in membership purchases, including auto-payment responsibilities and potential early cancellation fees. It is my responsibility to keep payment details current.
Personal Agreement: I declare that I am in good health for participation and will consult my doctor regarding my ability to participate in dance activities. I understand that Latin Roots is not responsible for recognizing or treating health problems.
Acknowledgement of Waiver: By participating in Latin Roots activities, I agree to these terms. If under 18, a parent or guardian’s consent is required.
Payment Authorization Form for Latin Roots
For One-Time Purchases:
I authorize a one-time charge for the amount indicated for goods/services I am purchasing. I understand that no notification will be provided unless the date or amount changes, at which point I will receive prior notice.
For Membership and Recurring Payments:
I authorize recurring charges every 4 weeks or as indicated in the membership description. This authorization will renew automatically and remain in effect until I cancel in writing at least 7 days before the next billing cycle.
I will not dispute transactions with my card company/bank as long as they correspond to the terms herein. I agree to provide updated billing information at least 14 days before the payment date. Failure to pay may result in late fees.
Cancellation Policies
Sessions: Must cancel 24 hours in advance to retain the session. Late cancellations result in a lost session. (Does not apply to Events and Courses.)
Events and Courses: Not refundable or transferable.
Memberships: Must notify Latin Roots in writing at least 7 days before the next payment date to cancel.
Refunds: There are no refunds, transfers, or extensions for unused sessions past the expiry date.
