Participation Waiver & Liability Release
I understand that participation in dance classes involves physical activity and carries an inherent risk of injury. I voluntarily assume all risks associated with participation and hereby waive, release, and discharge Latin Roots Dance and its instructors from any and all liability, claims, or demands for personal injury, illness, or property damage arising from participation in classes or events. I affirm that I am in good physical condition to participate and will notify instructors of any limitations.
I agree to follow all instructions provided and understand that my participation is voluntary.
